The patient’s cardiac status is assessed by sophisticated diagnostic equipments. It includes

12 channel Electrocardiography(ECG) – The Electrocardiogram is a pictorial record of the electrical activity of the heart, which gives doctors an idea of its functioning. Abnormalities in rhythm, size, structure, electrical pathways and blood supply of the heart can be detected with this simple investigation.

Exercise Stress testing or Treadmill test – Indication –The test is mainly used to estimate the likelihood and extent of coronary artery disease(CAD), and to determine prognosis and functional capacity, and the effects of therapy. Less common indications include evaluation of symptomatic recurrent exercise induced arrhythmias and conduction disturbances. The Stress Room is adequately equipped with emergency devices such as cardiac defibrillator, oxygen cylinders and life-saving medication.

24 hour Holter monitoring – The electrical activity of the heart is monitored continuously while the patient is ambulatory over a certain period ( usually done over 24 hours ), for evaluation of any rhythm disturbance , ischaemia etc.

Echocardiography and Colour Doppler – Echocardiography has become the standard diagnostic technique in modern cardiac care. Echo is often combined with Doppler ultrasound and color Doppler to evaluate blood flow across the heart’s valves.

Chest Pain Unit-CPU (Rapid triage of patients with chest pain) – Helps in the rapid assessment of patients who present with complaints of chest discomfort or other symptoms consistent with possible acute coronary syndrome. The CPU provides up to six hours of observation, serial ECG recording and biochemical cardiac testing including CK-MB and troponin. The patient is admitted if any of the following are recorded: >1mm ST elevation or depression in any two contiguous leads; T wave changes unrelated to posture or hyperventilation; arrhythmia; 2nd or 3rd degree heart block; or left bundle branch block or if cardiac enzymes are elevated.

Diagnostic procedures

Cardiac Catheterization and angiography – A diagnostic procedure which does a comprehensive examination of how the heart and its blood vessels function. One or more catheters is inserted through a peripheral blood vessel in the arm or leg with x-ray guidance. Typically performed along with angiography, a technique of injecting a dye into the vascular system to outline the heart and blood vessels.

Coronary Angiogram(CAG) – coronary angiogram, is the “gold standard” for the evaluation of coronary artery disease (CAD) and helps to identify the exact location and severity of CAD and is performed under local anesthesia. With the assistance of a fluoroscope a catheter is advanced to the opening of the coronary arteries and a small amount of radiographic contrast is injected into each coronary artery. The images that are produced are called the angiogram.

Radial angiogram (CAG through the hand)Angiography through the groin area is often painful for the patient to go through .The patient has to lie down on their back after the procedure which causes significant discomfort.. With radial approach( done from the hand) patient can stand up, sit down, use the toilet and even eat using the same hand immediately after the intervention.The patient can be discharged in 2.5 to 3 hours following a diagnostic angiogram done via the radial route.

Peripheral angiograms are done to detect blocks in the arteries which supply the blood to the head and neck or those to the abdomen and legs. Includes

PTCA( coronary angioplasty) – Angioplasty opens blocked arteries and restores normal blood flow to the heart muscle. A catheter is guided into the blocked heart artery from an artery in your leg or arm. A very thin wire is threaded across the blockage. Over this wire a collapsed wire mesh tube called a stent, tightly mounted on a special balloon, is moved over the wire to the blocked area. As the balloon is inflated, it opens the stent against the artery walls. The stent locks in this position and helps keep the artery open. The balloon and catheters are taken out.

TRANS RADIAL ANGIOPLASTY – Angioplasty is done from the hand and the patient is ambulant immediately after the procedure.Stable patients can be discharged on the same day itself.

PHARMACO INVASIVE THERAPY OF MI – Patients who are thrombolysed are taken up for angiogram and angioplasty within 3-24 hours. Peripheral angioplasty Balloon mitral valvuloplasty Balloon aortic valvuloplasty Balloon pulmonary valvuloplasty
